Munson Orthopedic Institute Brings New Technology and Comprehensive Model of Care

The Munson Healthcare Orthopedic Institute (MOI), a multi-subspeciality practice led by fellowship trained orthopedic surgeons, is now open and accepting new patients.

The MOI team of providers and staff brings decades of orthopedic experience both in the inpatient and outpatient setting with a passion for patient care. “Patients are at the center of everything we do at MOI,” stated Spencer Derenzy, Munson Healthcare Executive Director of Surgical Specialties and Orthopedic Services. “What you will find special about the MOI is the team atmosphere. We have created a smooth process and experience for the patients.”

Established to complement and expand access to orthopedic services in our region, MOI will focus on having subspecialized surgeons to treat a patient’s specific orthopedic problem or injury. Additionally, through partnerships and teamwork, MOI has aligned with Mary Free Bed to provide exceptional rehabilitation therapy services to patients. From pre-surgery therapy (prehab) to post-surgical treatment, physical therapy will be at the core of the patient’s road to full recovery.

From smart implants that transmit data to keep the clinical team updated on how a patient is healing to robotics, next generation technology is a priority for MOI. They are the first in northern Michigan to offer state-of-the-art Robotic Surgical Assistant (ROSA) knee-replacement surgery that provides a blueprint of a patient’s joint to offer the most precise and highly individualized treatment for patients. ROSA is available now at Munson Medical Center and Munson Healthcare Grayling Hospital.

Matthew Dubiel, MD, is fellowship trained from the Mayo Clinic in shoulder and elbow surgery. Dr. Dubiel will join the team in September 2022.

Peter McAndrews, DO, FAOAO, is an orthopedic surgeon fellowship trained in sports medicine and has been practicing medicine for over 20 years. A three-time Ironman finisher and lifelong athlete, Dr. McAndrews understands the importance sports medicine plays.

COVID CORNER

Evusheld Order Process
A reminder that providers are now asked to obtain patient consent when ordering Evusheld treatment.

When ordering Evusheld, providers are asked to complete the MHC Evusheld Provider Order and Patient Consent Form online then print and fax to the chosen site of service. Evusheld is offered in Cadillac, Charlevoix, Frankfort, Gaylord, Grayling, Kalkaska, Manistee, and Traverse City.

NOTE: For those offices that prefer to print the order prior to completion, please use the updated form as it has been redesigned for easier printing.

Patients will be contacted to schedule Evusheld infusion after all required documentation, including consent to treat, is received.

This is a one-time order. A new order is required for subsequent doses (to be given 6 months after previous dose).

Additional COVID-19 Vaccination Record Cards
Vaccination Record Cards for many recipients of COVID-19 vaccines are now full, especially for those over 50 years of age or immunocompromised individuals seeking additional boosters. When a vaccination record card is full, the CDC recommends:

  1. Complete a new card for the patient
  2. Staple both cards together
  3. Encourage the patient to photograph both cards in case they become separated
  4. Bring both cards to future vaccination appointments, travel, employment or any other purpose requiring official, universally recognized documentation.

Please note: Some providers have the option to provide the immunization record from the Michigan Care Improvement Registry (MCIR). The MICR record may not be acceptable for travel, employment, or any other purpose that requires official, universally recognized documentation. It is best practice to use Vaccination Record Cards.

VACCINE HESITANCY

How to Navigate Vaccine Conversations with Parents, Patients

Earlier this year, the I Vaccinate initiative (supported by the Michigan Health & Hospital Association) launched a provider toolkit website to help healthcare providers educate Michigan parents about the safety and effectiveness of all childhood immunizations. It also helps providers navigate difficult questions hesitant parents may have, especially as Michigan’s overall childhood vaccination rates have declined during COVID.
To help reach the goal of raising Michigan’s childhood immunization rates to national levels or above within 5 years, the I Vaccinate Provider Toolkit helps providers prepare for conversations, navigate and track difficult questions, and improve their ability to ensure their young patients are vaccinated on schedule or help them catch up on missed immunizations.

Providers can access the toolkit by clicking on “Request Access” on the login page and submitting their name, email, organization, and title.

COVID-19 Vaccine for Children
A reminder that the CDC now recommends that all children 6 months through 18 years of age should receive a COVID-19 vaccine. A handy At-A-Glance COVID Vaccination Schedules is available on the CDC website.

 MDHHS has also provided resources to support providers in having these conversations with parents.

Four General Principals of Motivational Interviewing
When speaking to parents and children about vaccine hesitancy, providers might find it helpful to employ motivational interviewing techniques. Below is an overview of the four principals of motivational interviewing. Look for more details, including examples, in future issues of Practice Pro.

Express Empathy

  • Express empathy through careful listening.
  • Explore concerns and benefits of the behavior change through non-judgmental curiosity.
  • Focus on guiding the conversation versus being the expert.
  • Support the patient’s autonomy and right to choose to accept or reject change.

Develop Discrepancy

  • Begin by asking open-ended questions to allow patients to explore their values.
  • Then highlight any discrepancy between a patient’s present behavior and their expressed values.
  • Change is more likely when the patient can make these connections rather than being informed of them by a clinician.

Roll with Resistance

  • Avoid trying to convince the patient of the right thing to do.
  • Trust that they are capable of change.
  • Use reflective listening when the client expresses resistance.
  • Ask permission to provide information and resist the urge to correct or argue with the patient.

Support Self-efficacy

  • Remember that patients are the experts in themselves.
  • Patients do better when they are encouraged to take an active role in their treatment.
  • A clinician’s role is to recognize the patient’s efforts towards change and highlight their existing strengths and resources.

Physician Web Scheduler: Save Time by Using Online Scheduling

Physician Web Scheduler (PWS) is used by offices to conveniently schedule close-to-home tests for their patients online. Instead of wasting time on hold or faxing orders, PWS allows your staff to immediately schedule at check out to ensure that important tests are on your patient’s calendar, which increases the likelihood of patients following through with their tests.

Recently, James Castillo, a scheduler for Traverse Bay Radiation Oncologists, took the initiative to conduct his own process improvement study that compared using PWS for online scheduling versus calling to schedule.

“PWS significantly reduces the time spent scheduling radiology imaging as compared to conventional transcription/fax/phone calls,” said James. “It also allows me to coordinate multiple appointments while the patient is still in our office, which improves patient satisfaction.”
